| China Airlines Flight 140 |
On April 26, 1994, China Airlines was due to land in Nagoya, Japan. The Airbus 300 was completing a routine flight and approach, however just before landing, the First Officer pressed the Take Off/Go-Around button (also known as a TOGA) which raises the throttle position to the same as take offs and go-arounds. When he attempted to correct the situation by manually reducing the throttles, he did not correct the pitch of the plane, which caused the plane to have a nose-high attitude. This nose-high attitude, combined with decreasing airspeed due to insufficient thrust, resulted in an aerodynamic stall of the aircraft. With insufficient altitude to recover from this condition, the subsequent crash killed 264 of the 271 people aboard. The crash which destroyed the plane (delivered less than 3 years earlier in 1991) was attributed to crew error for their failure to correct the controls as well as the airspeed. (references) |
| Cosmos 140 |
Cosmos 140 was a probable Soyuz programme precursor. It was the third attempted flight of the Soyuz 7K-OK model. The spacecraft experienced attitude control and excessive fuel consumption in orbit, but remained controllable. An attempted maneuver on the 22nd revolution still showed problems with the control system. It malfunctioned yet again during retrofire, leading to a steeper than planned reentry and a 300 mm hole being burned in the heat shield. Although such an event would be lethel to any human occupants, the capsules recovery systems operated and the capsule crashed through the ice of the frozen Aral Sea, hundreds of kilometers short of its landing zone. The spacecraft finally sank in 10 meters of water and has to be retrieved by divers. However, the mission was "good enough" for the next mission set to be a manned two-craft docking and crew transfer space spectacular. (references) |

| Interstate 140 (North Carolina) |
Interstate 140 in North Carolina or the Wilmington Outer Loop is a planned 27-mile (44 km) spur of Interstate 40 near Wilmington, North Carolina. Its primary function is providing a bypass for U.S. Highway 17 around Wilmington, which is reflected in the original name for the road project, the U.S. 17 Wilmington Bypass. The bypass is divided into three sections based on their estimated completion dates. The middle section, extending west from Interstate 40 to U.S. Highway 421 is 8 miles (13 km) in length and is scheduled to be completed by the end of 2005, 3 miles of this section from I-40 to NC 133 (Castle Hayne Road) opened in September 2005. The section extending east from Interstate 40 to U.S. Highway 17 near Scotts Hill is 7 miles (11 km) in length and is scheduled to be completed in June 2006. The final section, 12 miles (19 km) in length, extends west from U.S. Highway 421 to U.S. Highway 74 and U.S. Highway 76, and finally terminating at U.S. Highway 17. This section is not scheduled to begin construction until 2009 and several mitigating factors, such as a second river crossing, may delay its construction. Additionally, a proposed southern extension of Interstate 140 would extend the route 8 miles further southeasterly from U.S. Highway 17 to its terminus south of Wilmington back across the Cape Fear River using the proposed Cape Fear Skyway Bridge.
